Concealed Shipment Damage/Losses
Damage/loss claims must be documented upon delivery and reported directly to Mallinckrodt’s Customer Service Group at 800.325.8888 within 10 days of purchase. The U.S. Drug Enforcement Administration recommends that all customers open and inventory all shipments as soon as possible. Failure to report these damages/Iosses in a timely manner significantly impairs Mallinckrodt’s ability to properly investigate the cause of the damage or loss. Any damage or loss reported to Mallinckrodt more than 10 days after receipt by the customer will not be eligible for credit.
Shipments and Warehouse Facility
Our distribution center is located in our pharmaceuticals manufacturing and packaging facility at 172 Railroad Avenue, Hobart, NY, 13788. This shipping facility is a state-of-the-art distribution center which will enhance our daily shipping efficiency and provide better service for our customers’ product orders and scheduling needs.
Our distribution center ships products five days a week, Monday through Friday. Methadone products ship to treatment clinics Monday through Thursday.

Drug Safety
The Pharmacovigilance Department consists of healthcare professionals who are responsible globally for the collection, processing, and analysis of adverse events for Mallinckrodt’s Dosage Pharmaceutical, Contrast Media, and Nuclear Medicine product lines in compliance with regulatory requirements. Pharmacovigilance is defined as the science and activities relating to the detection, assessment, understanding, and prevention of adverse effects or any other medicine-related problem. The goal of our department is to minimize or prevent harm to our patients by identifying new safety concerns and/or hazards associated with Mallinckrodt products and to implement risk minimization measures when appropriate.
